We are looking for a Transition Mentor at Stoke Damerel Community College and Scott Medial and Healthcare College Campus.

Main responsibilities:
  • To support the successful transition of all students from primary to secondary school.
  • To attend meetings with Year 6 students/families prior to transition in order to collect and collate quality information that will support them and avoid any dips in learning. This includes the preparation and collation of data and records for students.
  • To use the information provided by primary school, parents and other agencies to create and regularly update student profiles and to disseminate this information as required.
  • To identify the students with poor literacy and numeracy skills before they enter the College and actions to support them are put into place, in collaboration with other key staff.
  • To be proactive in communicating with parents and responding to their queries through the transition period; throughout Year 6 and Year 7. To provide assistance and information for the Year 6 parent/carers of students transitioning to SDCC.
  • To aid the coordination of pastoral events related to Year 6 transition and Year 7. To plan a coherent package of transition activities to include Induction day and the start of the first term of Year 7.
  • To plan and implement an ‘Enhanced Transition’ programme for students.
  • To work with the Head of Year supporting the daily operations of the year group and the wider school community. To share the day-to-day responsibility with tutors, Head of Year and Assistant Head of Year for the uniform and equipment of students; including liaising with parents as appropriate.
  • To carry out administrative tasks related to Year 6 transition and Year 7
  • To use the Secondary Transition Portal to retrieve information relevant to the transition.
  • To support the Head of Year by providing pastoral support to year 6/7 students and parents.

You will need:
  • GCSE in English and Maths or equivalent at least level 4
  • Experience of working with young people in a paid or voluntary setting
  • Ability to relate well to children and adults
  • Ability to work in a team and as an individual
  • Good level of literacy
  • Understanding of relevant policies/codes of practice and awareness of relevant legislation
  • Experience of working in a customer facing environment providing general administrative support
