A fantastic opportunity for Wellbeing Assessors to join a well-established team who supports adults through a strengths-based approach and a range of short term and long-term interventions with varying levels of complexity.
 
Wellbeing Assessors will routinely undertake Social Care assessments for adults and carers, complete reviews, care and support plans, be involved in the completion of mental capacity assessments, continuing health care decision support tools, and contributing to safeguarding enquiries.
 
We are looking for resilient individuals who are passionate about advocating a good quality of life for adults with learning disabilities and their families, with a keen interest in promoting independence and wellbeing.
 
You will approach situations in a creative and pragmatic way, supporting individuals to find solutions, focussing on strengths and options available in the community.
 
The right candidate will be able to manage a caseload with autonomy but within a very supportive team.
 
You will be able to work in partnership with a range of other agencies such as health, education and the police and work with individuals who are often facing complex social and family situations.
