Service Care Solutions are currently recruiting for a Young Persons Advisor to join a supportive team at Wolverhampton Council. As a Young Persons Advisor, you will play a pivotal role in the Reach Leaving Care Team, supporting and guiding young individuals in their transition to independent living. Make a difference by providing personalised advice, fostering growth, and advocating for their future success.. You will manage and oversee the delivery of care planning and pathway planning whilst completing statutory duties in relation to care leavers up to the age of 25.
Location: Priory Green Building, or any suitable location within the City of Wolverhampton
Pay: £17.59 p/h
Hours: Full time
Duration: 3 month Contract with the potential of being extended
Responsibilities:
- Hold responsibility for a caseload of young people who are Care Leavers (relevant and former relevant young people).
- Complete Pathway Plans and Reviews for young people as required.
- Support Foster Carers and residential staff to prepare young people for leaving care and to continue to provide young people with practical, emotional and social support as they make the transition from care to independent adulthood.
- Continuously evaluate and review the service provided for Care Leavers to ensure that standards of care are maintained and that they experience good outcomes.
Requirements:
- Full UK Driving Licence & access to own vehicle
- Relevant qualification at NVQ Level 3 Children and Young People's Workforce or equivalent in social work, youth work, or related field.
- Experience of working with children and young people in either a social care, educational or youth work setting.
- Experience of report writing.
- Experience of assessing situations and recognising results.
- Understanding of legislation and guidance regarding Children and families including leaving care.
