Our Croydon service is a vibrant, collaborative team, comprising a dedicated lineup that includes a service manager, consultant psychiatrists, medical professionals, compassionate nurses, attentive healthcare assistants, empathetic recovery staff, passionate volunteers, efficient administrators, and supportive peer mentors.

Where: Croydon
Hours: 37.5hrs per week
Salary: £27,449.53 - £31,529.41 plus Outer London Weighting (£2,080.07)
Your Impactful Role: As a Young Persons Transitional Worker, you'll craft bespoke interventions, placing young individuals at the heart of your approach.
Your goal? To empower, reduce risks, bolster resilience, and cultivate safe, happy, and healthy lives.
Key Responsibilities Include:
Expertly coordinating multiple transitions for young adults into adult support systems
Delivering tailored, age-appropriate care and treatment for 18–25-year-olds
Conducting comprehensive assessments, implementing psychosocial interventions, and integrated care planning
Vigilantly identifying and responding to potential safeguarding needs
Fostering a supportive, holistic environment for positive health and wellbeing outcomes
Your Stellar Qualities:
Experience in substance use intervention
Exceptional communication skills, both written and verbal, coupled with strong IT proficiency
Proactive, autonomous, excellent time management, and priority setting abilities
Proficiency in mental health interventions and practices
A collaborative, supportive team player with robust interpersonal skills
Champion for change, putting the needs of service users at the forefront
