Job description
The Youth Care Co-ordinator with liaise with clinicians to identify appropriate referrals and develop the referral pathway in Primary Care for 1118-year-olds. To offer support and guidance to young people who may be struggling with their emotional and mental health wellbeing. Signposting young people and families to appropriate services. To support young people to increase, access to community services, employment, education, and training opportunities.
To increase awareness of and strengthen the young persons support network. To work using person-centred approach in developing relationships with benefit agencies, housing, and training establishments to enable opportunities for young people. Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN. Able to support people in a way that inspires trust, confidence, and motivating others to reach their potential.

Act in ways that support equality and diversity and recognises the importance of peoples rights in accordance with legislation, policies, procedures, and good practice. Valuing people as individuals and treating everyone with dignity and respect, consideration and without prejudice, respecting diversity and recognising peoples expressed beliefs, preferences, and choices in working with others and delivering appropriate services. To provide support and assistance regarding health promotion appropriate interventions and onward signposting as required.
To forge a culture of partnership between primary care and specialist agencies and specifically acting as the point of contact for young people. You will be working alongside the multi-disciplinary PCN team together with local partners to support the smooth delivery of the service.
