Lead role in establishing, implementing, monitoring, developing and evaluating discharge management for individuals with learning disabilities who have inpatient status in learning disability, and mental health inpatient facilities.
To collaborate with the Learning Disabilities inpatient service, primary health care teams, Community Learning Disabilities Teams, Social Work and Voluntary Care Providers, in order to improve the patient journey for patients who have mental illness, forensic issues and severe and enduring behaviour that challenges services.
Key responsibility for influencing and facilitating patient/carer centred integrated care pathways, from referral to inpatient services, throughout the assessment, formulation and treatment process in order to facilitate an effective discharge management process.
This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
