Job Title: Activities Co-ordinator

Responsible to: Service Manager

Responsible for: Management of care and support duties and safe service delivery

Hours of Work: 10-6 Mon-Fri (Will be required to work 1 Saturday and 1 Sunday in each 4-week period)  

Location: YMCA Together Hestia House, South Liverpool

Job Purpose

To assist in the provision of effective, high-quality care and support services to the people we support who meet the threshold for statutory care options in our residential setting.


Hours of Work

Primarily you will work 10-6 Monday through Friday. In each 4-week period you will be required to work 1 Saturday and 1 Sunday. Weeks in which you work a Saturday or Sunday will be treated as flexi weeks. This means if you work on a Saturday or Sunday in a specific week, you can choose which of the Monday to Friday days you don’t work.  


Duties and responsibilities

The specific areas of responsibility are set out below. These are subject to regular review in order to meet the changing needs of the organisation and the environment in which it operates.


Specific Duties

Facilitating & Planning Meaningful activities 

  • Support with planning activities and meaningful activities, person centred plans as part of assessment and induction 
  • Support residents to maintain their independence, to achieve their goals and targets by meaningful activities 
  • Have input into resident risk assessments and ensure awareness of risks in line with YMCA Together policy and procedure including positive risk 
  • Ensure understanding of specific person centred needs like cognitive ability and mobility so that needs can always be adequately met. 
  • Work collaboratively with all other statutory and non-statutory agencies and if there is an unmet need work alongside team leaders and service manager to ensure correct pathways are followed up. 
  • Keep up to date records on support planning systems MainStay/DAVE and in any resident’s files 
  • Support residents with activities including life skills like scheduled cleaning duties including cooking & room cleans 
  • Lead in the planning and delivery of group work and activities throughout Hestia House 
  • Ensure that all residents receive some form of social activity each week and residents receive one to one time, particularly those that do not wish to participate in group activities. This should include opportunities within the home and the wider community.  
  • To formulate and maintain a stimulating programme of activities for groups of residents and for individuals whilst considering residents particular likes and dislikes as appropriate. To produce a weekly programme of activities and social events and ensure it is advertised within the home and communicated to all residents.  
  • Ensure the activity and social programme incorporates evenings and weekends.  
  • To liaise with relatives and staff regarding special individual requirements, such as birthdays and anniversaries, cultural and religious festivals and celebrations, and to be aware of the religious calendar that may affect activity pursuits, relevant to the ethnic mix of residents.  
  • To familiarise and connect with all key community groups whose particular expertise would make an invaluable contribution to the care home.  
  • To support any volunteers in their role and ensure they receive appropriate induction, training, supervision and support.  
  • To promote and enhance the existence of the home in the community and encourage the use of communal spaces within the home for groups to meet and hold events.  
  • To regularly assess the effectiveness of the social events and activities programme through uptake and gaining feedback from residents 
  • To work as an active team member with your colleagues within the home to ensure the highest standards of care are achieved.  
  • To undertake any reasonable request which facilitates the resident’s right to choice, dignity and respect.  
  • Motivate and challenge the people we support so they identify and realise their goals 
  • Plan and lead on regular resident meetings 
  • Accurately and promptly record and report all complaints, accidents and safeguarding concerns or disclosures 
  • Provide clear, accurate and good quality information in respect of all residents in line with agreed procedures ensuring that appropriate information is shared with the line managers 
  • Demonstrate creativity in overcoming obstacles and a commitment to working with and providing support to vulnerable people. 
  • To undertake any other duties, within your area as requested by the service manager

Health and safety

  • To liaise with all emergency services when necessary 
  • Ensure at all times the safety of yourself and others on the premises in which you work 
  • To report, log and communicate all incidents to the appropriate people 
  • Ensure adherence to YMCA Together visitor’s policy and procedure 
  • To adhere to all risk management protocols. 

Teamwork

  • To share information and expertise with the team, and working together to provide a high-quality service 
  • To contribute to the formulation of policy and procedures in relation to care and support services 
  • Attend to all enquiries and pass messages onto relevant people 
  • Develop excellent and robust relationships, protocols, contacts and service level agreements with referring agencies, statutory services and wider stakeholder  
  • To attend and participate in regular team meetings to ensure good communication. 

Professional Practice and Development

  • Attend regular supervision sessions with the Service Manager in accordance with the supervision contract
  • Maintain good professional practice and ensure ongoing development, through use of supervision and training
  • Respect the confidentiality of Residents using the service and be responsible for the security of personal and confidential information
  • Carry out all these duties within the organisation’s safety guidelines
  • Ensure personal records are correctly maintained
  • Ensure effective communication with staff within YMCA Together
  • Work flexibly to meet the changing needs of the organisation
  • Undertake any other duties that may be reasonably requested
  • Flexible and positive approach with all team members

  • Develop and/or maintain excellent and robust relationships, protocols, contacts and service level agreements with Mersey Care, referring agencies, statutory services and wider stakeholders

Relationships

The post holder will be required to have contact with a variety of people during the course of their work. The context of this contact will vary from formal ‘Key working’ to informal networking and relationship building with the Local Authority and other relevant agencies.

Annual appraisal and regular work review with line managers is an operational requirement, as is attendance at weekly support team meetings.

It is recognised that the organisation is operating in a dynamic environment and as such this job description may be varied in consultation with the post holder to reflect evolving needs of the business and changing nature of the job.


Person Specification – Activities Co-ordinator

Qualifications

Essential

  • Qualification or working experience in Residential, Care or Support Services  

Desirable

  • A recognised qualification in Social Care, Health or mental health
  • Qualification in Substance misuse, welfare rights
  • IT qualification such as CLAIT, ECDL, ITQ.

Experience

Essential

  • At least 1 years’ experience of working within a Residential, Care or Support setting or similar and a working knowledge of clients support needs including mental health issues, offending, drug and alcohol misuse, working with partner agencies, safeguarding and life skills. 
  • An understanding and experience of providing and delivering meaningful activities to residents. 
  • Facilitating and planning group and one to one sessions 
  • Planning engaging person-centred activities 
  • Facilitating/Chairing resident meetings 

Desirable

  • Experience working in a CQC registered care setting
  • Experience of coordinating three-way meetings
  • Experience of working with end of life
  • Experience of resettlement of an individual into independent living.
  • Experience of providing personal care

Knowledge

Essential

  • A working knowledge of providing meaningful activities
  • A working knowledge of Health & Safety issues
  • A good understanding of the mental capacity act and the ability to make best interest decision and explain reasoning
  • A clear understanding of complex needs support issues such as mental health, homelessness, substance misuse and offending
  • Knowledge of Adult safeguarding protocols
  • Understanding of how to deal with challenging and chaotic individuals
  • A knowledge of how to provide dignity in care

Desirable

  • Knowledge of health and social care interventions
  • Knowledge of Psychological Informed practice
  • Knowledge of health intervention and harm reduction

Skills

Essential

  • The ability to engage groups and work one to one to with residents to build trusting relationships

  • To provide effective and consistent care and support planning with the people we support
  • Excellent IT, communication, interpersonal, interviewing and assessment skills
  • Excellent administration skills and the ability to accurately and concisely record information and write reports
  • Plan and deliver workshops and sessions to residents, staff and partner agencies
  • Demonstrate creativity in overcoming obstacles and a commitment to working with and providing support to vulnerable people
