Key Responsibilities:

• Conduct comprehensive assessments of older individuals' mental health needs.
• Develop and implement care plans to support the mental well-being of clients.
• Provide ongoing support and intervention tailored to individual needs.
• Collaborate with multidisciplinary teams to ensure a holistic approach to care.

Requirements:

• Qualified Social Worker with registration with Social Work England.
• Experience in working with older adults, particularly in mental health.
• Strong commitment to safeguarding and promoting the well-being of vulnerable adults.
• Excellent communication and teamwork skills.
