Adults Social Worker - Preparation For Adulthood

Our Client, Warrington Borough Council, is seeking an Adult’s Social Worker to join their Preparation for Adulthood Team.

Responsibilities of this role:

• Working with young people in Sheffield (aged 14-18+), as they approach adulthood, to support their transition from children's services into adult services.
• Completing social care assessments (in line with The Care Act) and implementing support plans to meet any eligible care needs that a young person may have, as they turn 18, experience of both children's and adults disabilities services is an advantage.
• Knowledge of the Mental Capacity Act and the Children's Act and how this can impact on key decisions for any young person they may be working with, and will be expected to utilise Universal Services to look at promoting independence for the young people of Sheffield.

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have minimum of 2 years’ experience.
