JOB TITLE:Advanced Nurse Practitioner Interview dates: Thursday 11th April 2024 The Fakenham Medical Practice We require an experienced, enthusiastic and motivated practice nurse to join our expanding friendly team. We are a highly skilled and experienced practice team and provide a supportive learning environment. The Fakenham Medical Practice is an eleven-doctor dispensing practice providing care to around 15,000 patients in and around Fakenham in North Norfolk. We work from modern, purpose-built premises.
The partners and staff have a shared belief in the delivery of high quality, traditional personal family healthcare. Patients interests are kept very much to the fore and there is a strong commitment to develop patient services. To promote a multi-disciplinary team approach, we share our premises with other providers including a pharmacy, adult social services and local community services e.g. physiotherapy and midwifery.
The practice is part of North Norfolk Clinical Commissioning Group and has a branch surgery at Walsingham. We are a training practice for GP Trainees. Practice Mission Statement Fakenham Medical Practice is committed to working with compassion and understanding to provide all its patients with accessible, responsive and efficient services that deliver good, safe and effective care. Main purpose of role To work in accordance with practice guidelines and protocols to provide high quality nursing care and meet the needs of patients.
To project a positive, friendly and professional image of the practice to patients, carers and other visitors to the practice. Key relationships Partners, salaried doctors, trainees, Operations Manager, Nurse Manager, Nurses, HCAs, all other Fakenham Medical Practice staff, patients, carers, Patient Participation Group members and other community providers. Job Duties and responsibilities: To assist the medical personnel in the care of practice patients to include diagnosis, treatment, preventative care, screening and patient education. Providing assessment, screening treatment services and health education advice.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with a minor illness. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Undertake care for patients in their homes as required by the Partners.
Provide regular care to patients in residential and nursing homes, ensuring all patients have up to date care plans. Work with vulnerable patients to help reduce the need for unplanned hospital admissions ensuring all patients have up to date care plans in place. Provide nursing treatment to patients in participation with general practitioners or independently within agreed protocols. Provide general and specific health screening to the practice patients (within the agreed protocols) with referral to general practitioners as necessary.
Be competent and confident in managing people with long term health conditions. Assess diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug- based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Delivering a quality service Recognise and work within own competence and professional code of conduct as regulated by the NMC. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
Deliver care according to NSF, NICE guidelines and evidence-based care. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate. Evaluate patients response to health care provision and the effectiveness of care. Pathological specimens and investigatory procedures Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.
Administrative and professional responsibilities Support staff development in order to maximise potential. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice. Take a lead role in planning and implementing changes within the area of care and responsibility Contribute to the development of local guidelines, protocols and standards. Prioritise own workload and ensure effective time-management strategies are embedded within the culture of the team.
Ensure accurate notes of all consultations and treatments are recorded adequately on the computer. Ensure acute completion of all necessary documentation associated with the patient healthcare and registration of the practice. Ensure collection and maintenance of statistical information required for regular and ad hoc reports. Liaison Maintain effective liaison with other agencies and staff concerned with patient care and all other disciplines within the practice, with appropriate regard to confidentiality.
Professional Development Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development and DOH requirements. General Staff must behave in a professional and discreet manner at all times. Full details of conditions of employment are shown in the contract of employment. Evidence and Research To evaluate and audit the practice of self and others, selecting and applying a broad range of valid, reliable and appropriate evaluation approaches and methods.
To maintain an active interest in research and participate in research studies independently or in collaboration with other health professional to improve nursing care. Confidentiality In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential Patient and professional confidentiality is of prime importance.
Any breach of confidentiality can result in immediate suspension/dismissal. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data. Quality The post-holder will strive to maintain quality within the practice, and will: Alert other team members to issues of quality and risk. Assess own performance and take accountability for own actions, either directly or under supervision.
Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. Communication The post-holder should recognise the importance of effective communication within the team and will strive to: Communicate effectively with other team members, Chief Executive, Practice Manager and GPs. Contribution to the Implementation of Services The post-holder will Apply practice policies, standards and guidance. To communicate politely and effectively with patients, carers, colleagues and other contacts, recognising peoples needs for alternative methods of communication e.g.
translation services and responding accordingly. In this way, through good communication and working collaboratively with colleagues and other agencies to support the provision of a seamless co-ordinated multidisciplinary service. At all times, to maintain the highest standards of behaviour, to comply with and follow practice and CQC policies, protocols and procedures, including information governance, confidentiality, health and safety, infection control, equality and diversity and to report any risk/breach or suspected risk/breach immediately.
