Job description

The primary focus of the role is to work as an advanced, autonomous practitioner providing patient-centred clinical care, encompassing the skills of assessment, examination, diagnosis and treatment of patients. The Advanced Practitioner will work across the 4 pillars of practice – Leadership & Management, Clinical, Education, Research and Development, as outlined in the Framework for Advanced Nursing, Midwifery and Allied Health Professional Practice in Wales 2010 (FANMAHPPW). The post is intended to provide improved outcomes for Eating Disorders Service patients by improving timeliness and consistency of care.
The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role. Guided by health, organisational or broad occupational policies, but in most situations the post holder will need to establish the way in which these should be interpreted and advise relevant stakeholders accordingly. Will be expected work at an advanced clinical level within their level of competence and to work flexibly as part of the team.
Will be expected to act as a practice leader, managing their own workload and work across professional, organisational, agency and system boundaries to improve services and develop practice. Participate in networks, locally, regionally and nationally. The post holder will assess and manage risk and proactively challenge others about risk.
To work within the Eating Disorders Service (SCAMHS as well as transition cases to adult) undertaking assessments, interventions, medication reviews, medical management and provide leadership and support for team colleagues. Provide mental health assessment, treatment planning and support for patients known to the Eating Disorders team within hospital and community settings. Develop and implement care plans for patients.
To undertake the role of Care and Treatment Plan (CTP) coordinator for a small number of patients, taking responsibility for initiating planning and review of care plans and risk assessments. Be able to assess, diagnose, treat/manage, evaluate, refer and discharge. Carries out non-medical prescribing in line with legislation. Manage complexity, uses critical thinking and analytical skills to underpin sound judgement, and ethical decision making.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
