Job responsibilities The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a. Primarily act as a decision maker using provided evidence (history examination & investigations) to manage patients with long term conditions in acute and chronic situations, working autonomously within the clinical team. b.
Manage & run the practice nursing service, acting as a role model, supervising colleagues in the nursing team and supporting them to take on increasing practice nursing skills. c. Work closely with the GP team to deliver care in an efficient and patient centred way, according to recognised guidelines and protocols d. Supervise other colleagues as required, including junior doctors, and liaising with other team members including admin and attached PCN and community colleagues.
e. Develop, implement and embed health promotion and wellbeing programmes f. Manage patients with a range of acute and long-term chronic medical conditions, providing subject matter expert advice, to include diabetes, asthma, copd, cardiovascular disease g. To manage cervical screening, follow up referral and recall h.
Implement and evaluate individual specialised treatment plans for chronic disease patients i. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health j. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF k. Provide wound care (ulcer/Doppler, etc.) l.
Provide specialist services to include contraception, HRT etc. m. Undertake the collection of pathological specimens n. Process and interpret pathology and other test results as required o.
Provide decision making on the management of chronic diseases following reviews, delivering patient care as necessary, referring patients to secondary/specialist care as required p. Maintain accurate clinical records in conjunction with extant legislation q. Ensure SNOMED CT codes are used effectively r. Maintain chronic disease registers s.
Manage and treat patients based on CVD risk assessment following NHS health checks t. Work within and ensure management of an effective call/recall system u. Be responsible for the implementation of an effective immunisation programme v. Chaperone patients where necessary w.
Prioritise health issues and intervene appropriately x. Support the team in dealing with clinical emergencies y. Recognise, assess and refer patients presenting with mental health needs z. Implement vaccination programmes for adults and children aa.
Be an extended and supplementary prescriber, adhering to extant guidance bb. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required cc. Contribute to practice targets (QOF, etc.), complying with local and regional guidance dd. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.) ee.
Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual) ff. Support the clinical team with all safeguarding matters in accordance with local and national policies gg. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately hh. Deliver opportunistic health promotion where appropriate
