School Nurse Practitioner

Undertake duties as allocated by the SCPHN School Nurse. Build and develop good working relationships with named schools and the wider multi-disciplinary team. Make contact with children, young people (CYP) and their families in a variety of community settings e.g. schools, home, children centres, youth clubs Deliver planned programmes of health promotion to CYP either through one to one contact, group work or through delivery of school based PSHE lessons Assist in the delivery of the Healthy Child Programme 5-19 In the absence of the SCPHN may undertake young peoples Drop In clinic in secondary schools Provide enuresis advice and continence assessments Identify and support CYP through health needs assessments who have unmet health needs, provide support and targeted interventions and where necessary refer to specialist services Take appropriate action in cases of suspected or actual child abuse, working within Blackpools safeguarding childrens code of practice and procedures. In the absence of the SCPHN may attend child protection conferences/core groups as required and participate in 4x 4x4 supervision. Undertake health needs assessments for children who are looked after Assist in research and service audits Supervise junior members of the 4-19 team as directed by the SCPHN SN undertake junior staff appraisals. Participate in meetings as appropriate Be prepared to be flexible within contracted hours Following experience, be prepared to complete the Specialist Community Public Health (School Nursing) Post Graduate Degree. Follow and engage in the School Nurse Practitioner Development programme and engage in reflective and self directed learning. In accordance with the needs of the service, undertake other duties as required
