Job description
The post holder will act as an Advanced Practitioner Prison Healthcare Dietitian Level 1 delivering dietetic support to the Primary Care service in Healthcare in Prison (HiP). This will include treating patients with a range of conditions e.g. diabetes, gastroenterology conditions, malnutrition as well as planning and delivering health promotion activities. The post holder will coordinate and develop the Nutrition and Dietetic Service for the GP Practice in Healthcare in Prison. Work in partnership with patients, Trust staff, Northern Ireland Prison Service (NIPS) staff and others internal and external to Healthcare in Prison to achieve optimal nutritional care for clients. Act as a source of specialist expertise for professional colleagues in this specialist area and ensure the dissemination of up to date information on nutritional care for therapeutic intervention and health improvement to all relevant staff. Work closely with the AHP Lead Healthcare in Prison, the Lead Dietitian and others regarding the ongoing development of the Dietetic Service across Healthcare in Prison, evaluating the effectiveness of dietetic intervention and proposing areas for improvement. Ensure accurate, consistent and evidence based health improvement nutritional advice is provided to the prison population by Trust staff, NIPS staff and community, voluntary and other statutory organisations. Assess and treat patients who may have complex and/or chronic presentations and determine independent nutritional diagnosis/diagnoses and resultant diet therapy indicated.Work independently as an Advanced Practitioner Healthcare in Prison Dietitian Level 1..
