To lead and co-ordinate the professional clinical and pharmaceutical input into the safe and cost effective management and development of the utilisation of medication in conjunction with Trust emergency care and re-admissions service and lead consultants ensuring their safe and secure use and minimising the risk of inappropriate use. To provide expert advice in the designated area to all directorate undertaking risk management, financial management and clinical governance activities and undertake prescribing to deliver medicines optimization and safe and effective discharge To monitor performance of pharmacists working in this area, to undertake clinical supervision and internal and external teaching for all staff To co-ordinate clinical service delivery within the safe and sustainable care project focused on front of house and ambulatory care pathways. To link with antibiotic lead pharmacist to deliver effective antibiotic utilisation and initiation to reduce the incidence of Hospital Acquired Infections.
