Shift Pattern:?7.5 hrs each day (30 hours over 4 days) Mon-Fri. Day off can be negotiated.

Pay Rate:?£19.10 per hour

Manchester University Foundation Trust are currently looking for an experienced Band 6 Physiotherapist to work within the community for Manchester Local Care Organisation.

The successful applicant will have experience using outcome measures and goal setting with patients. You will also demonstrate an appropriate level of clinical skills in Physiotherapy, experienced in person-centred assessments, as well as developing, implementing and evaluating programmes of care.

The post holder will also be responsible for assessment, implementation, planning and evaluating the delivery of patient care. The post holder will work within a multi-disciplinary/multi agency service, supporting patient flow out of the D2A Pathway 1, Crisis Response and Community beds/community services and provide continuation of therapy intervention and rehabilitation for patients in their own home.

To be successful in applying for this role you will need:

• NHS experienced Physiotherapist

• HCPC Registration

• Experienced in supporting Band 5 therapists and therapy support workers

• Experienced in person-centred assessments

• Driver with access to own vehicle
