Job description

We have an exciting opportunity to join our Keynsham service as a Band 6 Senior Community Nurse based at our Keynsham Health Centre location.

You will join a caring and supportive team of Community Nurses and Healthcare Assistants where individual development and progression is key and teamwork is at the centre. You will also work alongside the Reablement team, Community Matrons and specialist nurses in a multi-disciplinary atmosphere. The service is located right at the end of the high-street with a range of shops and Sainsbury’s and Tesco situated nearby. The service also has its own dedicated car park.

The  service hours typically being 8.00am - 5pm Monday to Friday with one in every 4 weekends. Your focus will be to offer advice and support to housebound service users in their own homes to promote their well-being and independence. You will also be offered a flexible role where you can work your own diary to provide continuity of care. There may also be the opportunity to support in a weekly leg ulcer clinic.

Main ResponsibilitiesWithin this role, you will:  


responsible for contributing to the holistic assessment, planning delivery and evaluation of care to patients within their own homes.
implement, monitor and maintain high standards of care delivery at all times.
care for patients and their families in the way that you believe is best
deliver our high service standards, sharing best practice, and actively improving the way we work. 
The purpose of the role is to manage the district nursing team. The post holder has overall responsibility for all aspects of service user management in the team, planning day to day care and problem solving for the team where necessary, influencing changes to clinical practice and being a role model. The role involves communicating with the multidisciplinary team and attending MDT meetings

The Ideal CandidateYou will be a NMC/1st level registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that you should have strong communication and organisation skills and a flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey.  You will have a full UK driving license with access to a car for work purposes.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
