Job description


The successful applicant will be dynamic, highly motivated individuals, with excellent interpersonal and communication skills. As a Special School Assistant Practitioner/Nurse Associate will be working as part of pro-active skill mix team who are committed to delivering high quality services.
The service provides support to children, young people and their families within a progressive universal model of service delivery dependent on assessed need. The service offers every child/family a programme of screening, developmental reviews and information and guidance to support parenting and healthy choices as defined by The Healthy Child Programme 0 – 19 years and up to the age of 25 years for children and young people with Special Educational Needs.
The Children’s service provides support to children, young people and their families within a progressive universal model of service delivery dependent on assessed need. The service offers every child/family a programme of screening, developmental reviews and information and guidance to support parenting and healthy choices as defined by current School Health drivers.
The post holder will work as part of a team to promote and maintain the health and wellbeing of children and young people with mild to moderate and severe learning difficulties which could also include those with complex medical needs. This will include providing clinical care within the school environment.

Contribute to the provision of a high-quality child health service.
Work collaboratively within the team and with primary and community care colleagues to deliver local and national health objectives.
Work with the team to undertake workload planning and delegation of work to other team members, taking into account competency levels.
Undertake holistic assessments of children / young people and families in line with local and national guidelines.
Support the team to develop new ways of working to provide a responsive service to clients.
Support and empathise with children and families in time of need.
Support the team to develop multi-agency partnerships and joint working to facilitate public health objectives in line with the Trusts objectives.
Work within a public health competency framework to address public health priorities and health inequalities.
Participate in the mentorship and assessment of a range of learners and in the preceptorship and support of new colleagues.
Maintain stock as required.
