Please see the attached documents for full job description JOB PURPOSE The post holder will assist the Registered Clinician in providing skilled nursing care to patients in need of specialist palliative end of life care. They will provide safe, effective and responsive nursing care to patients and their loved ones in a supportive, caring and compassionate environment. Contribute to the day to day operational management and ongoing development of nursing services by undertaking a range of additional responsibilities, appropriate for level of competence as delegated by the Lead Nurse. The postholder will be expected to be able to communicate effectively and empathetically with patients and their loved ones.
KEY ACCOUNTABILITIES & RESPONSIBILITIES Clinical Practice Leadership/ Service Development Training and Education Audit and Research KEY WORKING RELATIONSHIPS The main internal and external contacts the post holder will work with includes: Patients and their loved ones To be able to communicate effectively and sensitively with patients and their loved ones during highly stressful and emotional situations. Ensuring effective and honest communication considering the level of understanding, culture and background Line Manager and Colleagues - Communicates clearly and regularly discussing, reporting patient care. Working collaboratively to share information. Hospice Multidisciplinary Team and Volunteers Work collaboratively across all disciplines and departments to provide timely quality care and information to patients and their loved ones ensuring care is effective and coordinated.
Senior Management Team and the Registered manager Work in collaboration to ensure patient quality and safety is a high priority. External agencies such as Echo Hub, SCFT and Primary Care, Hampshire Community Trust, Continuing Health Care, St Richards Hospital and Nursing Homes- Establish and maintain good working relationship with colleagues, so as to provide a comprehensive service to patients and loved ones to enable good liaison and share best practice. Students across all disciplines providing a welcoming environment Trustees and donors ensuring a positive message is communicated about clinical services JOB SCOPE Decision making level The postholder will be able to work independently providing safe, competent, and ethical client care, under the guidance of registered healthcare professionals, in the delivery of palliative care to patients and their loved ones, utilising established protocols and procedures. They will undertake delegated tasks in which they have been trained and assessed as competent Financial resources To have the delegated responsibility of maintaining stock levels and using stock responsibility.
To be responsible for the care and safe keeping of equipment issued for personal and team use and to check and report any defect or loss. Be responsible for the care and safe keeping of patient belongings Other resources Personal responsibility for ensuring the security of the Hospice is maintained, escalating concerns as necessary in line with St Wilfrids Hospice guidelines. A working knowledge of IT systems particularly patient electronic records and Microsoft Office. People management Assist in creating the conditions that help the team provide mutual care and support and pride in achievement Be able to receive and act upon challenge and feedback and take action to improve own performance Challenge colleagues in a constructive and respectful way Participate in the induction of new staff and students Support volunteers to fulfil their role Legal, regulatory and compliance responsibility Responsibility for ensuring adherence to organisational policies and procedures Actively engage in the organisational Safety learning Event systems and actions.
Practice within Caldicott and Data and Information Governance principles Awareness of legislation and policy relating to safeguarding children and vulnerable adults (e.g. The Children Act (1989 and 2004), The Children and Families Act (2014), The Safeguarding Vulnerable Groups Act (2006) Awareness of Health and Safety responsibilities particularly when working alone in highly emotional and stressful circumstances. Follow guidance on Duty of Candour: openness with patients and their loved ones about anything that places them at risk of harm or causes actual harm. STANDARDS AND EXPECTATIONS Policies and Procedures All Hospice employees are expected to follow policies, procedures and guidance as well as professional standards and guidelines.
Confidentiality/ Data protection You should be aware of the confidential nature of the Hospice environment and/or your role. Any matters of a confidential nature, relating to patients, carers, relatives, staff or volunteers must not be divulged to any unauthorised person. You should make yourself aware of the requirements of the Data Protection Act and follow hospice procedures to ensure appropriate action is taken to safeguard confidential information. Health and safety You are required to take reasonable care for your own health and safety and that of others who may be affected by your acts or omissions and you should ensure that statutory regulations, policies, codes of practice and safety and good house-keeping rules are adhered to, attending training as required.
Safeguarding and Mental Capacity Act All employees have a responsibility to safeguard and promote the welfare of adults, children and young adults. It is essential that all safeguarding concerns are recognised and acted on appropriately in line with the policies and training. You must ensure you always act in the best interests of any person lacking mental capacity. Infection control All employees have personal responsibility for Infection Prevention and Control practice.
You should ensure you are familiar with, and comply with, all relevant Infection Control policies and training for minimising the risk of avoidable Infection.
