This is an exciting opportunity to join our existing Transitions Team providing wellbeing support and intensive client work across the Leeds & Wakefield district. The transitions team supports young people that have previously engaged in child and adolescent mental health services (CAMHS) and are now over 16 years old and need further support in order to help them live an independent, healthier life. This support is based on the young person’s needs and whilst typically focuses around mental health, emotional and wellbeing support, can also extend to variety of areas such as housing, financial, career and social support.
This role offers a unique opportunity to work with a young person for a minimum of 5 months to identify their needs, work with them to develop an appropriate support package and help them in their journey to recovery. This holistic approach means the role can be extremely varied and lead to working with the individual across a number of settings and with a number of different agencies and organisations to best support that young person. Offering such a variety of support means this role will have more opportunities for learning, training and personal development to ensure the successful applicant feels confident and competent to support the young person across a variety of areas.
Wherever possible for sustainability and support, we are keen to make community links and partnerships, therefore the successful applicant will have the exciting role of continuing to build on new and existing relationships across Leeds & Wakefield and support our Senior Transitions Worker to map and build on our knowledge of community provision and support as well as developing our signposting, community engagement and onward referral links.
This unique role will have a case load of young people and be part of the wider transitions team delivering across West Yorkshire regions.
