About the Role

Colne Valley PCN has an opportunity for a Cancer Care co-ordinator to join our primary care network (PCN), to support our 6 GP practices across the Colne Valley area to assist in re-shaping primary care across our communities.

The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service. There is a requirement to work from surgeries along with working from home.

The main duties of the role include:

  • Be the first point of contact for all newly diagnosed cancer patients.
  • Act as the main contact for patients who have been referred via the suspected cancer referral pathway from primary care into secondary care. Ensuring cancer referral safety netting.
  • Provide patients with high quality, easy to understand information to assist them in making choices about their care
  • Liaise with patients by letter, telephone, or face to face appointments to ensure the relevant information and support is given to enable patients to attend at hospital and all relevant appointments related to their diagnosis.
  • Listen to a patient’s needs from the point of referral to newly diagnosed and beyond, through answering queries and sign posting to the relevant services.
  • To be able to manage a patient needs appropriately documenting all consultations within the patient’s notes.
  • Work with patients, their families, and carers to provide appropriate care is in place and that expectations are managed.
  • Contact and organise clinical reviews for all patients with a new diagnosis with a GP where appropriate.
  • Complete a Cancer care review with patients via telephone consultation or face to face, recording an accurate and concise consultation within the patient’s notes using SystmOne clinical systems.
  • Liaise with appropriate clinical professionals to maximise patient needs to include identifying patients to the Gold Standard Framework (GSF) list.
  • Build effective relationships with each PCN member practice and their staff.
  • Work collaboratively with the current clinical leads, clinical pharmacists, pharmacy technicians, other care coordinators and social prescriber link workers, through peer support and as part of the wider team.
  • Build effective relationships with local system partners such as: cancer alliances, hospice outreach team, district nurses and public health.
  • Work proactively with local secondary care sites to understand the discharge process and be able to positively input into processes that optimise the patient’s journey.
  • Keeping up to date with National/Local Cancer Strategies and input these updates into the Colne Valley PCN meetings.
  • Run weekly clinical system searches for newly diagnosed cancer patients, contacting patient to arrange appointments.
  • Contribute to increasing uptake of national screening programmes throughout the Colne Valley Primary Care Network.
  • Contribute to the evaluation of the service, collate, and input timely data and suggest/implement service improvements.
  • Attend and contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the service performance
  • Produce performance and quality improvement reports as requested by PCN Leadership.
  • Working with the Lead Patient Care Coordinator, input into the management of patient complaints and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events.
  • Attend surgeries across the PCN attending meetings and working with other PCN or surgery teams
  • Attend regular meetings with the lead Cancer Care co-ordinator and cancer lead
  • Undertake any other duties deemed appropriate by the lead patient care coordinator, PCN operational manager or PCN cancer lead.
  • Make appropriate referrals from the completed Cancer care review recording within the patient notes and complying with relevant data privacy and consent.
  • Seek regular feedback about the quality of service and impact of care coordination on referral agencies.

About the Candidate

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups;
  • Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification;
  • Strong listening and communication skills;
  • Familiar with local resources and services, including how to access them;
  • Motivated by helping people, with care and empathy;
  • Able to deliver person-centred support in a non-judgmental way;
  • Experience of completing holistic person-centred care planning assessments.

About Us

The Colne Valley Primary Care Network is a collaboration of six GP practices serving approximately 54,000 patients, from the Kelvedon, Finchingfield, Halstead, Earls Colne, Coggeshall, and Hedingham areas.
