Job Description
An opportunity to be part of an exciting new way of providing patient care within a GP Surgery setting. The Care Co-ordinator Team work alongside all other departments to improve patient access within the Surgery and other Healthcare providers.
The right candidate will be able to effectively look after each patient that contacts the Surgery seeking healthcare, guidance or just a sympathetic ear. As a smaller Surgery we pride ourselves on being able to offer a personalised approach to our patients care and are committed to ensuring that our patient population needs are met during challenging times.
Main Responsibilities
Day to Day-
- Ensure that each patient has their needs met efficiently and correctly
- Making appointments via telephone, in person and, in the future, an e-consult model
- Home visit requests & organising community healthcare with the aim to reduce hospital admissions
- Interpreting results where appropriate and relaying these to patients and other healthcare providers
- Patient record input- to be able to enter administrative and clinical information via our operating system
- Administrative tasks to support the dedicated Practice Administration Team
- Greet and assist patients with queries, equipment loans and reasonable requests.
- Collect payments from patients for non-NHS services
- Support the Practice Clinical Team where necessary and signpost appropriately
Long Term-
- Participate in and interpret Surgery audits
- Ensure that needs are being met within specific patient groups with the view to improve access and reduce inequalities.
- Liaise with other departments to deliver a cohesive and effective patient experience
- Be part of the Surgery Vaccination programmes
- Coordinate care for patients with long term and chronic conditions
- Supporting patients with immediate care needs
- Work alongside the Clinical Team to support residents in Care Home settings
Personal/Professional development
The post-holder will participate in any training programme implemented by the practice as part of this employment, such training to include:
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work
