JOB SUMMARY
Work with colleagues within the Practice to identify and manage appointments for a stable caseload of patients with long-term conditions, liaising with other health professionals as required, to deliver personalised care & be the first point of contact for these patients.
The Care Coordinator looks for opportunities to improve access to appointments for planned care and the management of long term conditions. They will collaborate with others in the PCN to identify ways in which appointments can be maximised to make the most out of every patient contact.
The role will be supported by the GP’s to discuss patient related concerns and to follow appropriate safeguarding procedures (e.g. abuse, domestic violence and support with mental health).
MAIN TASKS AND RESPONSIBILITIES
The Care Co-ordinator will:
· Work with colleagues within the GP practice and PCN to identify and manage appointments which support planned care
· Be a central point of contact for patients with long term conditions
· Proactively support ‘call and recall’ processes for the Practice
· Work closely with other PCN care coordinators to support call and recall processes
· Work proactively with the Practice pharmacist to support medication reviews and patient bookings
· Ensure all coordinated activity is documented and coded accurately
· Champion the use of MS Teams, proper coding and other best practice standards
· Champion the filling of appointments in shared clinics, proactively inviting in patients and booking in to make best use of appointment time
· Use AccuRx Self Book to send text messages and booking links to relevant patients to improve access and ensure filled clinics
PROFESSIONAL INTEGRITY
· The Care Co-ordinator will be expected to undertake developmental training to support the evolution of this new role.
