HOME CARE SERVICE CO-ORDINATOR – JOB DESCRIPTION
Job Title: Service Co-ordinator
Responsible to: Lead Co-Ordinator
Hours: 37 per week
Salary: £25,240 pa inclusive off on call allowance
Annual Leave: 37 days (inclusive of 12 days public holidays)
MAIN OBJECTIVES OF THE POST:
· Plan, coordinate and monitor service visits and educational escorts
· Co-ordinate geographically to maximise utilisation of resources
· Link care workers to Service Users using a person centred approach
· Liaise with Management to ensure cover is available i.e. sickness, absenteeism or holidays etc.
· Liaise with Service Users and workers to ensure an efficient service provision.
· Keep accurate records on the system and work with the office team to ensure all data is up to date and relevant.
· Prepare care worker fortnightly schedules
· Ensure Carers and Care Worker rosters are completed on a timely basis.
· Communicate effectively with other home care staff
· Participate in team meetings, planning meeting etc.
· Participate in the out-of-hours on-call rota i.e. dealing with any co-ordination responsibilities and liaising with the back-up on-call Manager with regards to any Social Care emergencies.

2. Quality Assurance
· Achieve agreed standards of service delivery in compliance with relevant legislation and National Care Standards.
· Be aware of and comply with the Scottish Social Services Council Code of Practice.
3. Administration duties
· Record information in line with the organisation’s Confidential Policy and the Data Protection Act.
· Participate in any other administrative duties as required by Management i.e. filing, photocopying, minute taking etc.
4. Health and Safety
· Ensure a healthy and safe working environment, and the best use of equipment, by implementing and adhering to policies and procedures.
5. Learning and Development
· Participate in internal and external learning opportunities, as directed.
· Actively participate in and be committed to the supervision and appraisal processes, in line with organisational policy.
6. Communications
· Promote the aims of the service and GEECC.
· Represent GEECC effectively at local level, liaising with individuals, stakeholders and communities.
7. Other Requirements
· Undertake any other tasks as may be reasonably requested.
· Participate in the Home Care service’s on call system as required.

Experience
· Previous experience within a Home Care Service or similar.
· Ability to demonstrate a successful track record within a similar role.
· Knowledge of Health and Safety and Quality Assurance
· Employment within regulated social services.
Education, Qualifications and Training
· Good working knowledge of IT to include Word, Excel and PowerPoint.
· Registered with professional body, i.e. SSSC or able to meet the requirements.
· Educated to SVQ3 or above.
Skills and Knowledge
· Good organisational, communication and literacy skills.
· Ability to work on own initiative and prioritise workload.
· Ability to meet deadlines.
· Experience of using a rostering system
Other
· Ability to work flexibly which may include evenings and weekends.
