Hays Healthcare are actively recruiting Registered Mental Health Nurses to work as a Care Coordinator for Early Interventionin Psychosis in Nottingham. The EIP team is a community-based service which operates Monday-Friday 9am-5pm. The post-holder will be required to travel across Nottinghamshire to support the needs of service users, their family members and carers. The use of digital technologies to support service need and delivery are likely to be required within this role.
Your new role
Working as a Care Coordinator within the Early Intervention in Psychosis Team providing evidence-based specialist mental health assessments and interventions that are socially inclusive and recovery-focused, in line with national standards for people aged 14-65 experiencing first episode psychosis (FEP) or an at risk mental state (ARMS) for the development of psychosis.
The role involves the timely assessment of service users referred to the Early Intervention in Psychosis (EIP) Team, the provision of associated evidence-based interventions, including Family Interventions, either on an individual or group basis to those supported by the service and the use of standard outcome measures to monitor recovery using the HoNOS, QPR and DIALOG.
What you'll need to succeed
- To be trained to be trained to degree / diploma level in either nursing or occupational therapy.
- To develop expertise in the delivery of Family Interventions for Psychosis which is underpinned by theory.
- To effectively manage a personal caseload and maintain robust communication with service users, carers, family members, other members of the MDT and any external agencies involved.
- To undertake specialist EIP assessments and deliver evidence-based interventions in accordance with the service remit and national standards.
- To collaboratively develop care plans with service users and identify key interventions to support service user recovery.
- To act as a Care Coordinator for identified service users within the Care Programme Approach (CPA) framework and ensure compliance with all CPA requirements as per Trust policy.
- To encourage and support service users to collaborate in interventions and their recovery.
- To encourage informed decision-making and the development of skill to support recovery free from mental health services within the client group.
- To undertake and record comprehensive risk assessments, identifying modulating factors and devise and implement risk management plans for all identified risks.
- To have understanding &/or experience of the impact of serious mental health problems and the ability to use engagement skills to work with service users
- To have an understanding of the role of physical wellbeing in mental health recovery
- Ability to carry out interventions with service users within community settings, working closely, with the multi-disciplinary team
