Position Overview: We are looking for a dedicated Care Team Leader to oversee and coordinate the delivery of care services to thier clients. The Care Team Leader will be responsible for leading a team of caregivers, ensuring the highest standards of care are maintained, and fostering a supportive and collaborative work environment.
Key Responsibilities:
- Supervise and mentor a team of caregivers, providing guidance, support, and ongoing training as needed.
- Coordinate client care schedules and assignments, ensuring appropriate staffing levels to meet client needs.
- Conduct regular assessments of client needs and develop personalized care plans in collaboration with clients, families, and healthcare professionals.
- Monitor the quality of care delivered by the team, conducting regular evaluations and implementing improvement strategies as necessary.
- Act as a liaison between clients, families, caregivers, and management, addressing any concerns or issues promptly and effectively.
- Ensure compliance with company policies, procedures, and regulatory requirements related to care delivery and documentation.
- Participate in the recruitment and onboarding of new caregivers, conducting interviews and assisting with orientation and training programs.
Qualifications:
- Proven experience in a caregiving role, with a minimum of [2] years of experience in a supervisory or leadership position.
