Primary job function

ASC Case Managers play a crucial role in coordinating care and support for residents (and their carers and families) who contact or are referred to adult social care from the community. The team provides the central community facing access point for adult social care enquiries and safeguarding concerns. Working as part of a multi-disciplinary team of social workers, occupational therapists, Case Managers and support workers/advisers, Case Managers will manage a varied caseload working with professionals across the team to ensure a seamless service to individuals with a range of needs.

Leading quality conversations with residents and referrers as directed by professionals within the team, Case Managers carry out a range of social care tasks including screening and triage, initial assessment, support planning and case coordination to an exceptional standard. Case Managers support people to identify their strengths and networks, delaying and preventing need for new or additional social care intervention wherever possible to enable people to continue living life in the way that they wish.

Key responsibilities

Our aim is to create a health & social care workforce that works seamlessly together to improve outcome for residents. This will include an integrated point of access which supports effective and efficient multidisciplinary decision making and planning, to reduce delays, avoiding duplication and bridge gaps, whilst ensuring the delivery of the right support at the right time to optimise independence and confidence.
The post holder will exemplify best practice in working with adults in strengths based and person centred way, connecting people with informal, community based support wherever possible. You and other team members will be fully committed to working in partnership with residents, carers, community based organisations and other key professionals involved in enabling people to be as independent as they possibly can.

The post holder will be part of Adult Social Care and could be based in one of the following teams:
The post holder will be part of a wider Adult Social Care team, further detail of the teams can be found in the Target Operating Model.
Resources and Financial management

1. Ensure that any commissioned packages are cost effective, innovative, using strength-based practice values and focussed on recovery
2. Always ensure, that use of resources is linked to clear impact and measureable outcomes for service users, and that the need for support is kept under continual review and changed appropriately
3. Comply with all resource and finance governance processes, cost controls and income maximisation in an ever-changing environment, fluctuating demands and priorities.

Person specification

You should demonstrate on your application form how you meet the essential criteria. Please ensure that you address each of the criteria as this will be assessed to determine your suitability for the post.

1. Qualification credit Framework level 3 (QCF) in health and social care or willing to train to achieve this.
2. Experience of working with adults in Social Care/Health/Voluntary Community Sector setting.
3. Being solution and resolution focused.
4. Listening to and working with local people.
5. A working knowledge of relevant legislation and guidance relating to the provision of services for adult client groups.
6. Knowledge of the effects of ill-health and disability with particular reference to race, gender and culture.
7. Good written and oral communication skills, including report writing and case recording.
8. Partnership working skills, working with stakeholders and as part of a multi-disciplinary team to deliver improved outcomes for residents.
9. Skills in negotiation, mediation and advocacy in order that each service user can achieve personally identified outcomes.
10. Making effective decisions.
11. Working together.
12. Ability to attend and participate in/lead reviews, case conferences and other meetings as appropriate.

This role will require you to obtain an Enhanced satisfactory clearance from the Disclosure and Barring Service
