To provide social work consultation and contribute to informal and formal teaching and training for members of the multi-disciplinary professionals working within care pathways in collaboration with the other team members via MDT meetings and other sessions, as appropriate. To take the lead on comprehensive psychosocial assessment of needs of children and their parents/carers, gathering information from the child, parents, carers, the multi-professional team, and other agencies, taking into account the rights of the child and the family to confidentiality, and bringing to the attention of the multi-professional team, any issues which are likely to affect the welfare of the child and family. To represent the needs of CYP in the context of their families with other agencies including the Housing Department, employers, the Benefits Agency, schools, nurseries and further education establishments. To act as an advocate at appeals tribunals. To act as specialist adviser to other agencies to ensure that the particular needs of CYP with severe obesity and their families are understood and they have access to appropriate services. To use a range of verbal and non-verbal communication tools to communicate effectively with children, their parents, carers and extended family members. To communicate effectively with colleagues, medical and non-medical staff on social issues around patient care. Maintain accurate and up-to-date documentation of all interventions with CYP and their families, taking account of their rights to access records under the Freedom of Information Act.
