SalaryReal Staffing are seeking an experienced interim Children's Continuing Healthcare Nurse Assessor/ Case Manager to support a NHS Integrated Care Board on a remote basis.
Key Responsibilities:
- Assessing healthcare needs for children requiring ongoing care
- Collaborating with healthcare professionals, families, and caregivers to develop comprehensive care plans
- Monitoring and evaluating the effectiveness of care plans, advocating for necessary adjustments when needed
- Providing support and guidance to families navigating the healthcare system
- Ensuring compliance with regulatory standards and guidelines
- Maintain accurate records and documentation
Requirements:
- Experience in complex CYP case management
- Qualified Children's Nurse with CHC experience
- Excellent communication and interpersonal skills
