The Children's Community Nursing Service (CCNS) is a dynamic team providing high quality nursing care to children and young people within the community, including the patient’s own home, special schools and respite facilities. You will have the opportunity to provide support within the special needs schools, liaising with tertiary centres to provide nursing care to the children and young people of Walsall. To provide an effective, evidence based Community Children’s Nursing Service to children with additional needs, disabilities, long term conditions, life limiting and life threatening conditions, and their families across Walsall through delivery of high quality nursing care
To manage a caseload of children and families with complex needs undertaking appropriate holistic assessment, intervention and evaluation through programme’s of care and support to meet identified needs and through active participation/ engagement with children, young people and their families.
To act as key worker/ lead professional for children and their families ensuring a coordinated and effective multi- agency approach

We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.
