An exciting opportunity has arisen for an enthusiastic Band 5 Community Nurse to come and join us in our Community District Nurse teams in Solihull.

The band 5 Community Nurse role is to assist/ support the District Nurses, lone working, delivery of high standards of nursing care to patients in the community by following prescribed programmes of care. The role involves working autonomously and a car driver/owner.

This post requires a motivational and enthusiastic individual who is passionate about patient care. You must be flexible, approachable, compassionate, motivated, and hardworking and be able to use your own initiative.

The post holder will be expected to workdays and weekends including bank holidays, 37.5 hours per week. Part time/Flexible working will be considered.

The service operates between 07.00 and 23.00 hours, 7 days a week and you will be required to work shifts between these times.

Main duties, tasks & skills required

To be responsible for the assessment of patients' nursing needs, taking into consideration the needs of family and carers and provide and maintain a high standard of skilled nursing care.

To work collaboratively with GPs and other professionals to ensure the best possible care is provided to patients in the community, avoiding unnecessary admissions and prolonged hospital admissions.

To recognise deterioration in patients' condition advising the caseload holder of any circumstances which give cause for concern.

To communicate effectively with patients, families and carers as required, intervening to overcome barriers to communication, using advocacy skills when necessary to achieve effective outcomes for patients who may have sensory impairment, confusion and/or aggression.

Participate in the promotion of healthy living and the prevention of ill health and disability through diagnostic screening, education and promotion of independence and self management as a priority of care.

To teach and support other qualified and unqualified staff including pre - registration and post-registration students.

To assist the caseload holder in planning and ensuring work is allocated on a daily basis, prioritising as the service requires utilising the dependency scoring tool.

To work in accordance with the Trust's professional policies and procedures to maintain professional standards. Practice in accordance with NMC Code of practice and maintain registration

Qualifications

Essential

  • Current registration as a First Level General Nurse

Experience

Essential

  • Knowledge in Safeguarding/ issues/procedures
  • Good IT skills

Desirable

  • IV Therapy Competent
  • Previous Community nursing experience
  • End of life care

  • UK Driving licence / car owner
  • Ability to work shifts
  • Flexibility to meet the needs of the service.
