Responsibilities
• Lead and coordinate the Discharge Planning Team• Support the coordination of discharges, including complex cases• Ensure the clinical team delivers a safe, effective, and caring service• Provide strong professional clinical and managerial leadership• Specialized knowledge in discharge planning
Requirements
• Current RN or registration with NMC, with relevant post-registration qualifications• BSc Degree in a relevant field (desirable)• Minimum of 5 years post-registration experience• Significant demonstrable experience within specialty• Experience managing resources and contributing to departmental budget• Skills in clinical decision making, communication, and interpersonal skills• Effective leadership and team building skills• Organizational skills, forward planning, and flexibility• Coaching, mentoring, and critical appraisal skills• Strong customer service skills and IT literacy
