Job description
Lead the team to deliver care that helps improve the health of the patient and wider community. Lead the team in demonstrating compassion through effective relationships based on empathy, respect and dignity. Develop own competence to deliver effective care and treatments through improving clinical/technical knowledge / skill, and expertise. To understand and support the various health assessments available for all patients utilising this service, maintaining a professional and caring approach at all times, To carry out the health assessment tests without supervision but with support of the GP if required as part of the assessment, To provide lifestyle and health promotion advice, based on information obtained during the patients assessment, To liaise with GPs, HEALTHFIRST team, Bookings, as appropriate
