Clinical Pharmacist, Falls Prevention Service With On Call

In order to meet the overall mission of the Trust the post holder will: Deliver a Falls Prevention Clinical Pharmacy Service, incorporating medicines optimisation and evidence-based fracture-risk assessment to support Falls Prevention within the Community Falls Prevention and Management Service to provide an enhanced level of care post-fall and improve patient outcomes. Accountable for carrying out fracture-risk assessments to identify patients at risk of fracture and prescribe bone-strengthening medicines as per national guidelines (NICE, NOGG, FRAX). Independently refer appropriately assessed patients to consultant geriatricians for radiological screening for osteoporosis and osteopenia. Work as a Clinical Pharmacist in the Community Falls Prevention and Management Service providing a medicines review and optimisation service, utilising independent prescribing skills, in order to identify any prescribed medications that may contribute to the risk of falls. .

Minimum of 2 years post registration experience of which 18 months (by 1st Aug 2024) must be as a band 6 hospital pharmacist.

MPharm or equivalent.

Member of the Pharmaceutical Society of Northern Ireland or eligible for membership.

Experience providing pharmaceutical care to older people with comorbidities and polypharmacy.
