Clinical Pharmacist Integrated Community Services

To provide specialised pharmaceutical advice and education to patients/carers and other health professionals Advise on therapeutic drug level monitoring for specific drugs To support safe and cost effective delivery of pharmaceutical services in bed based services and domiciliary care To provide assessment of patients ability to self-medicate and offer advice and guidance on self-management strategies for individuals To carry out follow up domiciliary visits to patients wherereviewing patients medicines and recommending processes to improve medicines optimisation ensuring all medicines are fit for purpose and appropriate Ensure medicines related incidents are reported ensuring that appropriate learning is implemented Support the delivery of a pharmacy service at Bolton One Pharmacy by providing pharmacy cover as necessary Assist in providing clinical training and supervision of pharmacy staff and other healthcare professionals
