Follow up to sessions by emailing out info attendees. Supporting staff in clinic by following up inputting to Nexus, highlighting missing info. Monthly review of Nexus to feed into MDT meetings Liaising with team and other multi-agency teams (internal and external) and arranging meetings where required Ordering of equipment/stores for the teams Ordering of patients own stores e.g. consumables Collect pharmacy for the teams Deliver prescriptions / medications to the patients home Clean / check safety of equipment used by nursing teams Demonstrate the use of equipment to families Conduct home visits to organise and deliver medications Patient observations if required by nursing teams Assist in clinics, distracting patients during invasive procedures Weight and heights and other specific tests required by the nursing teams Demonstrate the use of equipment trained to use to patients/carers To assist the specialist nursing teams to provide seamless care for their patients and families To assist the admin team to ensure the tasks are completed in a timely manner To assist with audits when needed Assisting with group education sessions Setting up and tracking patients attending specialist clinics and special school clinics
