Job summary
A wonderful opportunity has arisen for a Specialist B6/7 Development role to join the established Heart Failure team within Sussex Community NHS Foundation Trust supporting the East Locality.
The post holder will be supporting the management of complex Heart Failure patients with a collaborative aim in keeping patients well at home and to avoid hospital admissions where possible.
The successful applicant will be supported to complete relevant training, academic and clinical competencies in an agreed time frame whilst working autonomously as part of this dynamic team with patients in the community.
The post holder will support the Improvement of the patient's quality of life as well as their medical and nursing management through a combination of nurse led Heart Failure clinics and home visits, incorporating clinics with primary care teams and early follow up in line with local service specification.
As a Development Role, the successful candidate will be supported to complete Clinical and Training competencies within an agreed time Frame .

Main duties of the job

Person Specification
Qualifications
Essential
  • RGN first level nurse First Level degree or equivalent
  • Post graduate level degree or equivalent
  • Mentoring/supervisors Course
  • Extensive clinical experience of five years or more with 2 years at band 6 in Heart Failure
  • Advanced Physical Assessment course
  • Non-Medical Prescriber or willing to work towards

Experience
Essential
  • Case management experience
  • Evidence of recent management of Heart Failure patients
  • Evidence of up-to-date based knowledge and skills, particularly in in Long term Condition issues
  • Evidence of ability to maintain and monitor high standards of care
  • Evidence of professional development and knowledge
  • Community Nursing

  • Ability to work in flexible way and respond positively to change.
  • Ability to work autonomously and make own decisions
  • Ability to travel between sites, car owner/driver
