Community Hospital Pharmacist

To provide clinical pharmacist support to GP practices within North Hammersmith & Fulham PCN under the supervision and guidance of the senior pharmacy team. To provide pharmaceutical support to health care professionals (HCPs) and patients within the PCN. To conduct face-to -face structured medication reviews for patients in the clinic, practice setting, patients homes, within care homes or bedded services under the supervision of the senior pharmacist until assessed as clinically competent to conduct SMR independently. Support long term chronic disease management through polypharmacy review or medicines optimisation prescribing clinics under the supervision of the senior pharmacy team /specialist. Utilise clinical tools to proactively identify patients for structured medication reviews e.g. frail, recent discharge, history of falls.
