The team...

Our approach to how we deliver care and support is changing in Gateshead, having partnered with the National Development Team for inclusion's Community Led Support network, its an exciting time to be joining our Integrated Adults and Social Care Services team, with a refreshed 5-year strategy and action plan, we have great ambitions.

Adult Social care is at the frontline of tackling the inequalities in poverty and health that our communities face. Our Thrive Agenda aims to effectively address these issues and enable the members of our communities to live thriving lives. Working in Adult Social Care is not an easy job, however there aren't many that are more fulfilling when you can see the impact that you are having on the lives of the members of our community who are in need of support.

Community Led Support brings together a network of public, private and voluntary organisations who all want to improve the lives of our communities. The plan is to work together with our partners across localities and redesign the way we work with local communities.

About the role...

Working closely with the Community Led Support Programme Lead, you'll be joining a shared vision to drive impactful change together.

Ideally from either an Adult Social Care or Community Development background you will have excellent project management skills, be data driven and can work collaboratively with people, partners and communities to facilitate change.

A key task from the role is to lead on the development of our co-produced Prevention and Early Intervention Strategy in line with the introduction of Community Led Support, therefore you must be able to translate strategy into deliverable plans.
