Within our learning Disability Team, we are investing in creating the right conditions to ensure our Social Workers and Learning Disability Nurses, can work flexibly and are given the capacity to provide the excellent level of support and attention needed, to make a real difference to the lives of the people in our community.
You will have experience relevant to working within a community based role, supporting people with learning disabilities. As an autonomous practitioner you will manage a caseload providing assessment and interventions in relation to the health and social care needs of adults with a Learning Disability.
