Job Purpose

The Community Programme Health Lead will be responsible for the design, delivery and monitoring of a 12 month health programme. The project aims to encourage members of Yateley Industries and the wider community to increase their physical activity by experiencing a wide range of weekly physical activities. Through a personalised incentive scheme, focus will be on supporting individuals to create new lifelong habits and a sustainable fitness plan for when the programme has ended. The programme seeks to; improve physical health, reduce stress, increase social interaction and act as a signpost service to other community initiatives.

Key Responsibilities

  • Programme design, delivery and monitoring
  • Develop and manage a weekly programme of physical activities for our disabled residents and the wider community with focus on individuals who use Yateley Industries Community Hub services, those with disabilities, adults ages 55+ and those facing financial difficulty (though all adults are welcome to take part)
  • Ensure a variety of accessible opportunities are available both on and off-site.
  • Ensure that all activities and events are delivered within a safe and appropriate environment, following relevant policies and procedures.
  • The programme should be co-designed with participants – through group consultations and one to ones.
  • Feedback from participants should be gathered from a range of methods
  • Build and maintain strong relationships with local community groups, businesses, and organisations to create new health links and contacts.
  • Promote and advertise activities to the wider community.
  • Manage a budget ensuring all activities are delivered within budget.
  • Create monitoring systems to record each individual’s baseline of activity and the journey travelled by the end of the programme.
  • Help support the delivery of wider community hub activities
  • Incentive scheme
  • Create an award/ incentive scheme to help encourage attendance, commitment and long-term changes.
  • Sustainability
  • A key element of this role is to work with individuals to make long term changes in their physical activity. You will look to create a sustainability plan with each person to enable them to carry on the activity once the programme has ended. The budget and incentive scheme can be utilised to achieve this.
  • All personal plans will need to be well documented.

Note: Interest and knowledge for physical and activity and health is an important skill for this role, but a sports background is not essential.

Key Skills and Qualifications:

  • Experience of planning, delivering and evaluating Community Programmes or activities
  • A passion and interest in physical activity and health
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with a wide range of stakeholders.
  • Strong organisational skills, with the ability to manage multiple tasks and priorities.
  • Experience of working with disabled people or vulnerable adults would be advantageous.
