We are currently working with a Local NHS Trust who are seeking a Band 6 Community Psychiatric Nurse to join their Dementia Service in Hounslow.
What's on offer:
* Working hours: Monday-Friday, 9am-5pm
* Contract length: 3-6 month initial contract with the view of extension
* Location: Hounslow
* Rate of pay: £28p/h Umbrella (negotiable depending on experience)
About the role:
* Undertake assessments of individuals and their families in the designated care group with complex health presentations, including those service users presenting with higher levels of risk.
* Work alongside the service user to develop their care plan along the recovery principles.
* Assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.
Requirements:
* Registered Mental Health Nurse with a valid NMC Pin
* Previous experience working with dementia patients in the community
