Community Pastimes support small groups of people who may have a memory problem or diagnosis of dementia, with the same interests and hobbies to access the wider community. Our number one priority is making sure people are living life well and enjoying themselves to the full.
Location: The Heathers, Heather Grove, Hartlepool, TS24 8QZ
Hours: 32.5 hours per week (Monday to Friday) on a rota basis.
Job Purpose: To provide a wide range of group and individual activities and support to people with a dementia living in the community.
Salary: £11.31 per hour (£11.44 from April 2024)
Contract Period: Permanent
Key requirements:
-Valid current UK driving license and access to a car during working hours. This
role involves supporting people in Hartlepool & East Durham and travel within
these areas is essential.
Key Responsibilities
• To provide a range of one-to-one and small group, community activities for people as set out
in their outcomes and aspirations in the local community
• To be part of a network of services within Hospital of God in the community
• To provide transport to clients by the use of the employees own vehicle and ability to drive
safely and without limits
• To provide personal and social care to people in accordance with needs
• To ensure care plan and risk assessments are updated and referred to at all times
Key Duties
• To encourage and support clients to access community links by attending social events and
activities, shopping, walks, visiting friends and family, craft classes, museums as requested
by the client
o To ensure that clients safety and emotional well- being is paramount by attending to needs during outings
• Assist the clients with care in accordance with people needs
o If required, to assist with eating and drinking
o To administer medication if requested provided an agreement and consent to
administer prescribed medication has been signed by client/carer
o To report daily to the Manager any changes in the client’s condition
o To ensure the assessments of the support needs of clients are reviewed as required
• To ensure that the high standards of the Hospital of God are applied at all times
• To accept supervision from the Manager and at times other managers as appropriate
• To relieve stress placed on carers. Providing emotional support and help if requested, to improve care techniques always promoting carers self-esteem and respect.
o Be aware of carers needs and report appropriate information to manager
• To work with management to set in place values, policies and procedures that promotes the independence and well-being of clients.
• Adhere to the charities policies and procedures, promoting good practice at all times respecting confidentiality and ensuring dignity, privacy and respect is adhered to at all times
• Provide a professional specialised service to people with dementia
• Maximising a person’s independence in ensuring equipment needed is adequate and available
• Ensure the individual is safe and secure within the home environment, ensuring Health and Safety is promoted at all times
