Community Support Worker - Older Adult Mental Health Team

Permanent, Full Time

£23,456 to £29,933 per annum

Location: Chelmsford

Working Style: Anywhere worker

The Older Adult Mental Health Team Mid is based in Chelmsford and the team has an exciting opportunity for a Community Support Worker to join the team. The role will involve working with and managing a caseload of adults aged 65 years and over diagnosed with a functional or organic mental illness such as dementia; and their families both in the community by undertaking and writing assessments and developing discharge pathways from inpatient wards in accordance with the Mental Health Act 1983, Care Act 2014 principles and Mental Capacity Act 2005.

This is a dynamic role promoting innovate person centred and partnership working with adults, their informal carers and in collaboration with other statutory, and non-statutory services to support and promote their Mental Health, well-being and outcomes. An essential part of the role is to challenge stigma, discrimination and consider least restrictive approaches for adults and their situations through holistic assessments and considering positive risk management to promote outcomes and recovery.

The Community Support Worker will support the delivery of strength and asset based approaches promoting recovery and social inclusion with customers and families meeting agreed service standards.

Accountabilities

  • Undertake comprehensive needs-led assessments under the Care Act 2014 and prepare, implementing and the monitoring of care plans
  • Working with adults and their families/carers
  • Attending and participating effectively in supervision by team leader/senior practitioner on regular basis in accordance with Directorate policies
  • Co-working effectively with other professionals including attending multidisciplinary case discussions and team meetings
  • Recording information accurately in line with ECC policies
  • Expected to be committed to continuous workplace learning through organisations and attendance at social care forums and meetings
  • Ensure social care services are commissioned with best value and personalised outcomes in mind and ensure that social care funded packages are reviewed on a regular basis in a robust way.
  • To contribute to the commissioning and provision of effective after-care services, including those required under s117 MHA, Care Act 2014 and mental capacity act.

Knowledge, skills and experience

  • Some knowledge / experience of delivering public services in a multi-disciplinary environment will be beneficial
  • You'll bring a strong ability to build effective relationships with adults and their families in times of challenge and crisis.
  • You will have the drive and passion to want to make a difference, and a willingness to learn.
  • Ideally you will have some experience of activity-based work with adults in a social care setting and some knowledge of legislative and policy guidance applicable within social care settings.
  • You will ideally have knowledge of the local communities.
  • Evidence of ability to work as part of a team and ability to contribute to the development of services as part of a wider team
  • Proven ability to meet timescales and deadlines and maintain high quality service delivery.

Please note that the job role requires you to be mobile throughout an operational area. Therefore, the post holder will need to have a driving licence and access to a vehicle, or the ability to meet the mobility requirements of the role through other means.
