SalaryThe post holder will provide support to the One to One care in Care Homes quality scheme of work, ensuring that all One to One commissioned is being delivered in line with the policy and is clinically indicated. Ensure that the review of the health care needs of clients referred for One to One care (continuous observation) are identified and met. Collate and undertake comprehensive and co-ordinated assessments for decision making for NHS funded care specifically focussed on the one-to-one requirements for the person, reviewing all information including use of assistive technology or alternative methods of support. Assist in maintaining a database for all patients assessed as requiring One to One care in a care home and ensure proper quality assurance systems are in place. Develop strong working relationships with CHC nurse assessors, the Dementia Outreach team and providers to ensure One to One reviews are completed in a timely manner ensuring evidence of clinical rationale is available. work to provide professional support to homes to enable providers to manage falls risk and fully utilise tools such as assistive technology, cohort nursing, interval observations etc to ensure the safety and wellbeing of residents using the least restrictive possible options of care.
