Offer Culturally Specific Support to people from Black and Minoritised Communities whose needs are not currently met by other services, helping them to develop skills and strategies for greater independence.
Use an approach to delivering the service taking account of trauma informed care, person centered approach, and psychologically informed environment.
Establish relationship of trust with service users maintaining appropriate boundaries.
Supporting people to identify appropriate health, social care and housing services.
Referring and introducing people to services, accompanying them to appointments as necessary.
Take a partnership approach to working with service users including referring and signposting them to specialist agencies.
Assessing referrals made to the service ensuring they are a good fit.
Co-ordinate and review the planning and delivery of support to service users including actioning timely exiting of users who no longer need support.
Provide linguistic support when needed.
When necessary, take a persistent approach to advocating for service users including when other agencies are involved in providing support for the service user.
Using the outcomes measurement tools to assist people to identify and monitor the changes they want to make.
Report safeguarding adult and child concerns.
Work in partnership with stakeholders, landlords, Voluntary and Statutory agencies to access specialist services for service users and to promote the service.
Be a point of contact for other services working with individuals with multiple needs.
When appropriate be the key worker in coordinating activities in relation to housing support, mental health services and other areas of work to meet the needs of service users.
Work in collaboration and in partnership with other Culturally Specific Service team members, Steering Group and Emmanuel House services.
Undertaking caseload management and maintaining robust monitoring systems taking account of project data and expectations.
Where appropriate organise outreach surgeries to facilitate accessibility and promote the service.
Taking an active role, with other team members in the development of Emmanuel House’s policy and practice.
Attend team meetings and supervision.
Represent and promote the work of the Mental Health Support role and Emmanuel House Support Centre to other agencies and external meetings.
To adhere to the policies and procedures of Emmanuel House.
To be committed to the development of the best possible practice at Emmanuel House and encourage social inclusion.
Contribute to the evaluation of the service.
