Dementia Advisor

Are you looking for a fantastic temporary full-time role?
I am working with a thriving service, looking to recruit for a Dementia Advisor position in Ealing - W5 postcode.
Main Responsibilities include but are not limited to:
- To manage a caseload and give people with dementia, and their families or carers, access to the information and resources they need
- Carry out home visits to build trust and thereby assess and identify the social, mental and physical needs of people with dementia and their families or carers.
- Educate people with dementia and their families or carers, so that they can have choice and control over their current care plans and future care needs.
- Conduct ongoing monitoring of the social, mental and physical health of people with dementia and update GPs and other agencies following each review.
They are looking for:

Previous experience of working with older people or vulnerable adults

Experience of record keeping and report writing
Ability to manage own time and priorities within a caseload.
Hours of work are: 37 hours a week.
Paying £15.50 per hour
