DUTIES AND RESPONSIBILITIES Communication & Relationship Duties related to Service delivery Communicates effectively and is able to work with other members of the dietetics team to maintain working relationships Uses different methods of communication with individuals and their families to educate, facilitate and promote change. Reflects on barriers to communication and uses their knowledge of different methods to support and improve communication with individuals To communicate appropriate information to and from other service areas as required. Planning and Organisational Duties Schedules own diary to include domiciliary visits, virtual/face to face clinics, meetings, training and personal CPD with support from a registered professional where appropriate. Assists in the planning, delivery and evaluation of group health promotion/education/ physical activities for patients and other healthcare staff. Arrange weights with patients using appropriate equipment. Attends meetings and take notes as required including multidisciplinary, service development and team meetings. Order supplies of ONS samples for the team and liaise with company representatives. Responsibilities for Patient / Client Care including analytical and judgement skills. Manages an independent caseload of patients. This will require: Working within a variety of settings to assist in the delivery of this service including health centres and to domiciles throughout the area as required. This will include working with clients in groups as well as on a one to one basis Monitors clients nutritional care plans and reports any deviations to supervising dietitian or appropriate professional. Communicate any required changes to the care plan to the patient, relative or carer Involves patients and carers in the planning and delivery of care plans, respecting individuality, values, choice, privacy, dignity, cultural and religious diversity. Providing nutritional and lifestyle advice that is evidence based, taking into account medical, social and cultural factors. Attends regular supervisor meetings with registered professionals to discuss caseload. Undertakes clinical supervision as required by line manager. Accurately records all patient assessments and interventions in MDT record cards and others as appropriate, in line with the Services Record Keeping Policy and Data Protection Act Responsibility for Resources Financial, Physical, Human & information Participates in the training of pre-registration dietetic students on placement, and other students as required. May be required to contribute to the training of registered Healthcare professionals as required. Records and collect patient information both manually and electronically according to service standards. Maintain and orders department and external resources Responsibility for Policy & Service Development Working alongside a registered professional; Assists in service reviews and redesigns including the development of new ways of working. Contributes to the development of working relationships across health and social care sectors including voluntary organisations. Participates in the evaluation of some services. Responsibility for Research and development Maintains own CPD to meet the requirements of the appraisal and development review. Develops own knowledge and skills and contribute to the effective delivery of the dietetic service. Participates in the clinical governance and practice supervision programme Assists in the collection of data for use in audit and research programmes. Such information must be considered strictly confidential and must not be discussed or disclosed. Failure to observe this confidentiality could lead to disciplinary action being taken against you. Data Quality All employees are reminded about the importance of Data Quality and staff should make themselves aware of both departmental and corporate objectives for Data Quality. Data Quality forms part of the appraisal and objective setting process for staff responsible for data entry and data production; staff should ensure that they adhere to policies and procedures at all times. Failure to do so may result in disciplinary action being taken. Infection Prevention and Control: You must comply with all relevant policies, procedures and training on infection prevention and control. Safeguarding Children and Vulnerable Adults: You must comply with all relevant policies, procedures and training on safeguarding and promoting the welfare of children and vulnerable adults. Valuing Diversity and Promoting Equality: You must comply with all relevant policies, procedures and training on valuing diversity and promoting equality. Training: Managers are required to take responsibility for their own and their staffs development. All employees have a duty to attend all mandatory training sessions as required by the Trust.
