About Us:
We have three Adult Social Care teams based in St. Mary’s Hospital, Chelsea and Westminster Hospital, and Charing Cross Hospital. Our aim is to facilitate safe discharges from hospital with the appropriate level of care and without any unnecessary delays. We work collaboratively with the hospitals’ Discharge Hubs and other medical staff to ensure that people are assigned to the appropriate discharge pathway and that there is an emphasis on reablement and regaining independence wherever possible.
The Role:
You will support the safe and timely discharge of people from an acute hospital by working with LA housing partners to ensure people's care needs can be met.
This Tri-Borough role will be co-located within the hospital discharge teams at Chelsea and Westminster, St Mary’s and Charing Cross hospitals, working closely with hospital discharge teams and the discharge hub as the single point of contact for housing related issues impacting someone’s planned discharge.
You will lead the development and implementation of the hospital/housing discharge support and pathways for housing across Kensington and Chelsea, Westminster and Hammersmith and Fulham, working in close partnership with Housing and other key partners.

About You:

Our hospital discharge team is looking to recruit someone to work with local people to support their discharge or to prevent unnecessary admission to a hospital by working with local housing partners, health teams and our social workers. This is a unique role that will offer you the chance to work within a diverse and exciting team environment.
We are looking for someone who has good IT, verbal and written communication skills, and you’ll have exceptional negotiation and influencing capabilities, with the ability to get people to see your viewpoint and provide a great deal of comfort to people at their most vulnerable moments. You don’t need a social worker or other similar qualifications, but we do want you to have had experience of working within a Local Authority or Health setting, ideally with an understanding of housing-related issues.
