Job description

This is a fantastic opportunity for an experienced Registered Nurse (RN Adult) to join a pioneering community healthcare organisation for the Sutton borough area as a District Nurse Team Leader, supporting the delivery of high-quality integrated services that vulnerable patients can receive without needing to leave their homes.

As a District Nurse Team Leader, you will be responsible for providing professional leadership and management for members of the community nursing team. You will ensure that care planning reflects and accounts for the individual needs of your patients, that patients and their carers/loved ones are fully informed and educated about their care, and that your team has the resources, support and learning environment they need to maintain high standards of care in line with evolving clinical practice guidelines and regional health needs.

This is a permanent position for aDistrict Nurse Team Leader (RN Adult); both full-time (37.5h) and part-time contracts can be considered. Flexible shift patterns can be offered.

Person specification:

  • ( Essential ) Registration with the NMC as a Registered Nurse (RN Adult).
  • ( Essential ) Postgraduate qualification in District Nursing and NMC recognition as a Specialist Practitioner (District Nursing).
  • ( Essential ) Valid UK driving license and access to personal vehicle.
  • ( Essential ) Previous experience of nurse leadership/management.
